Evaluation Form for Girls
Print an Evaluation Form for each girl to complete at the end of Dream It, Be It. Then submit evaluation information by following the instructions and link below.
Participant Certificate
Download a Participant Certificate for each girl that completes the Dream It, Be It program.
Finished your project? Report your success!
Thank you for being local change-makers and hosting Dream It, Be It in your community! After you review the evaluation worksheet, submit your report online to receive your participation credit. We recommend submitting your report within one month after the project. However, all reports must be submitted by June 30th each year to receive recognition for participating in the program, and ensure your impact is measured.
